Make a difference supporting people experiencing or at risk of homelessness.

Join Communify’s HART 4000 team and help individuals find stability and sustainable housing through a Housing First and trauma-informed approach.

About the Role

As part of our Housing First and trauma-informed service, you’ll:

  • Conduct intake, assessment and case management with individuals and families
  • Manage a caseload of around 10–12 participants, plus drop-in crisis assessments on rotation
  • Develop and deliver client-focused case plans to achieve sustainable housing outcomes
  • Provide referrals and broker supports to meet immediate and long-term needs
  • Build partnerships with housing providers, government agencies and community services

About You

You’re passionate about social justice and skilled in supporting people facing housing instability. You’ll bring:

  • Relevant tertiary qualifications (e.g. Social Work, Community Services or related field)
  • Demonstrated case management experience using a strengths-based approach
  • Excellent communication, time management and problem-solving skills
  • Ability to respond effectively in crisis situations
  • Strong teamwork and collaboration skills
