We're seeking a dedicated and experienced Care Partner to join our growing team and play a pivotal role in coordinating high-quality, person-centred care for our participants.

About the Role

As a Care Partner, you will work closely with participants, families, carers, and healthcare professionals to develop, implement, and review personalised care plans that reflect each person's unique goals, needs, and aspirations. You will be responsible for coordinating services, monitoring participant wellbeing, and ensuring participants receive the right supports at the right time.

You'll act as a trusted point of contact, advocate for participant rights, and collaborate with multidisciplinary teams to ensure safe, effective, and high-quality service delivery.

Key Responsibilities
  • Conduct participant assessments and develop comprehensive, person-centred care plans.
  • Coordinate services and communicate effectively with participants, families, support workers, and health professionals.
  • Monitor participant wellbeing and changing needs, ensuring care plans remain current and effective.
  • Facilitate care reviews, case conferences, and ongoing care discussions.
  • Support participants to make informed decisions about their care and services.
  • Provide education, resources, advocacy, and guidance to participants and their supporters.
  • Monitor participant budgets and ensure services align with funding and program guidelines.
  • Build and maintain strong relationships with community organisations, health professionals, and referral networks.
  • Ensure accurate documentation, reporting, and compliance with organisational and regulatory requirements.
  • Contribute to continuous improvement initiatives and quality outcomes.
About You

To be successful in this role, you will have:

Essential Requirements
  • Experience in a Care Partner, Case Management, Care Coordination, or similar role.
  • Strong understanding of person-centred care principles.
  • Excellent communication, relationship-building, and conflict-resolution skills.
  • Experience working within multidisciplinary teams.
  • Strong organisational and administrative skills.
  • Confidence using client management and computerised information systems.
  • Ability to manage competing priorities and respond to changing participant needs.
  • Current Driver's Licence.
  • Current National Criminal History Check.
Desirable Qualifications & Experience
  • Qualifications in Aged Care, Disability, Community Services, Nursing, Health Services Assistance, Individual Support, or a related field.
  • Experience within aged care, disability, community services, or health sectors.
  • Current First Aid and CPR certification.
  • Experience with quality improvement programs and compliance frameworks.
