About the role
A fantastic opportunity has become available for an experienced Registered Nurse to join our team, comprising of home care services, aging in place and assisted living needs. You will be accountable for delivering high level care including clinical assessments and referrals, medication administration and wound management. You will also be undertaking comprehensive care assessments that enable older people under the new Support at Home program to live safely and independently at home, providing ongoing planning, monitoring, service coordination, education and direct nursing tasks with a strong focus on wellness, reablement, and culturally safe care. You'll work closely with clients, their families, and external providers to tailor support and foster strong, trusted relationships.
The Clinical Care Partner (formerly Case Manager) ensures that services address both clinical and non-clinical needs. The role combines advanced clinical judgment with care coordination, education, and participant advocacy. It also requires reviewing and updating budgets to ensure residents are billed correctly for services and make maximum use of their home care packages.
Key Responsibilities
Assess Client Needs: conduct initial and ongoing assessments to understand physical, emotional, and social needs, identifying risks and tailoring care plans with clients and families.
Care Planning: develop, review, and update person-centered care plans aligned with client goals and Support at Home funding.
Coordinate Services: organise in-home supports and liaise with external providers to ensure timely, quality care.
Advocate and Educate: support clients to understand their funding, choices, and rights, and advocate for access to appropriate services.
Budget Oversight: monitor individual budgets and guide clients on effective use of funds.
Relationship Building: build strong connections with participants, carers, and providers, supporting smooth transitions and ongoing care.
Compliance and Safety: maintain accurate documentation, uphold support at home and aged care standards, and proactively manage risks.
Skills you'll need to succeed
Demonstrated experience in care planning, case management and clinical assessment in aged care, community, or health settings
Minimum 1-2 years of experience in care planning and case management within healthcare settings
Solid understanding of the Support at Home Program, Aged Care Strengthened Standards, and Aged Care Statement of Rights
Strong communication, highly organised with advanced problem-solving skills
Competence in interpreting clinical guidance and escalating appropriate
Current registration with AHPRA as a Registered Nurse
Australian citizenship required for this position
